EMS—-Records

A
This chapter will deal with my latest age-80s medical/social problems interspersed with a report of my d saw aily progress that I believe are common to and relevant for everyone in modern society. I deal in issues that I have professional and personal experience with, and emphasize a problem-solving teaching mode. The presentation is not in a particular order; but the descending table of contents, (to be added) will guide the reader.

Now I face the challenge of late old age and imminent death.  For age 88, I am healthy, thanks to good parents, affluence, and good healthcare. But I also am developing serious medical problems due to my risky behavior and aging.  And I now face the irremovable problem of dying/death!
 The year opened for me with two importantly, worsening medical problems:
   1. Spinal atrophy involving lower extremities and sensory loss, all caused by collapse of  the 
Zlumbar-sacral vertebrae due to osteoporosis and from a lifetime of poor posture and frequent accidental falls; and, complicated, since first seen on CT scan in September 2021, by a tumor mass of the left side of the lumbar-sacral area. almost certainly malignant and with two apparent metastases to lungs. For my medical professional readers, here is a paste of the CT report, well before the tumor was seen:

Lumber bone MRI early 2017
Left-side deviated scoliosis is present. There are compression fractures in the L4 and L5 levels, with the irregular bone edge. Whole lumber bone shows severe deformity. At T1W,T2WI, there is a strong signal enhancing, especially in the lower part of L4 and upper part of L5, suggesting a Modic type II deformity. There is also Modic type I deformity in the upper part of L1 and L2. The whole lumber bones shows severe stenosis, creating the stenotic intervertebral foramen (especially at Th12/L1 and L1/2-L3/4 on the right side, and L4/5 and L5/S1 on the left side). These changes are similar to that in the previous study (in 2014, Dec 8). At the L3/4 level, there are perineural cysts near the intervertebral foramen. Likewise, other perineural cyst is present at the sacral bone level. Both renal cysts are also seen.
Final Diagnosis: Spondylosis deformans

Symptomatically, it has caused mostly low back pain but by 2019 a definite spinal syndrome has developed. 
    Most prominent and disturbing is an obvious atrophy of the anterior thigh muscles, chief of which are the quadriceps femoris It is causing great difficulty in lifting each foot from the floor and thus weakens walking and support of the body, and, as a result, I can now barely walk and only with a walk-wheelchair, without which I cannot even support standing up.
   Along with the muscle atrophy is a stocking&boot paresthesia extending to knees. It involves only touch; not pain &hot/cold . I also have anterior aching soreness pain extending down from waist to knees and bilaterally symmetrical.
 Comment on the syndrome: to be noted are its symmetrical right-left bilateralness, the sparing, in the paresthesia, and of pain&temp, and the hyperpathia in feet. With the other symptoms it suggests circumferential compression of the low spinal cord caused perhaps by the lumbar stenosis as part of my L-S vertebrae collapse.
   2. My other major medical problem is a chronic prostatitis and BPH (? on the BPH) which I have had since the 1950s. It has caused dysuria, frequency, and poor urination stream. There is evidence of a many-years developing low-grade obstruction at the prostate urethra causing a thick-walled, low capacity bladder with ineffective detrusor function and hyperactive internal urinary sphincter.
   On 1 Octr 2021 after a night of extreme small-pee urinary frequency, my urination blocked and I needed insertion of Foley catheter, size 16 in the ER, and the catheter was left in place and managed at home for a week. On 8 Octr, I was admitted to hospital urology to manage removal of the catheter and on 11 Octr, in hospital my catheter was pulled out and I was discharged home, 17 October.

Now I give insights from having catheter in bladder that should add to, not repeat, what a patient may already know or gets instructed. Catheterization  is sharply discomforting but takes less than 30 seconds.  You have it done because one can’t pee and needs it, agonizingly.  In rare cases it may be self-done. Once you have a catheter in you, you need to unplug and empty it every few hours unless it is attached to a collecting bag on thigh. While in place it can be mildly uncomfortable around the penis. I had mine in at home for 10 days w/o problem but I took antibiotic to prevent infection. Removing the catheter is more problematic than putting it in, not because of the actual removal—-that’s easy; just deflate the balloon and pull out. The problem is that your urination needs to be observed at least the next 24 hours to be sure you will be able to pee sans catheter again. 
   On 17 October, I discharged from hospital in wheelchair pushed by my helper Incidentally, I still can walk, barely, with exquisite discomfort on movement and weight bearing and joint movements.

   Now, it’s Tuesday, 19 Octr, 4 am and all is stable but I continue to be bothered by my usual pains that I shall discuss here from a teaching point of view.
  Each patient‘s pains have unique aspects. The commonality of pain is that it is a sensory experience that is on a pleasure/pain continuum from the extreme pleasure one gets on first use of an opioid to the low abdominal, suprapubic agony of, for example, obstructed urination with an overfilling bladder. My pains are a soreness of femurs and of the left hip joint and the left ischial tuberosity. (The bone beneath the part of the butt you sit on.) They wax and wane in no pattern. Occasionally they can be very bad, at my limit of tolerability. At worst, they need codeine 60 mg and acetaminophen 300 to 600 mg. Most of the time they need no medication and do not prevent sleep.
   My current plan: Even though my CTs show the tumor with metastasis, I need an exact tissue diagnosis of the cancer in order to receive palliative therapy. So I shall consult an expert at Japan’s National Cancer Center ASAP. 
   20 Octr at home, no problems. I keep myself medicated with 25 mg Sosegon (pentacozine, an opioid I have taken for 25 years for good feeling); codeine, 40 to 60 mg and acetaminophen 300 to 600 mg for pains, along with my usual beta-blocker, ACE-inhibitor and thiazides diuretic to keep my BP and resting heart rate within normal. I am eating well, small snacks at a time and urination is good but I am pretty much confined to my Simmonds chair with ambulatory needing wheelchair assist. (To continues

2008.10.07.2.EAJ: Aoki ( age  early 70s. In late 1999 worried about heart arrhythmia but turned out just Premature Contractions. In c.2003 had imaging for chest but nothing. Aoki san: at least 3 or 4 years ago he had >200 cholesterol. Takes a pill. Always appears weight-wise frail though actually energetic. This spring I note clearing of throat much. (Sign of enlarged left atrium?) He tells me today his chronic tinnitus is getting worse. 2008.07.07.1 to Yangon Myanmar for ten days for Platform. Back at work 07.16.3 and OQ. On 2008.08.06.4 had Dock exam. 2008.12.12  Since I got back late Nov he no longer takes lunch (This was temporary, by 2009 he is as usual though he eats carefully, taking small portion of rice bowl in Bamiyan – 2010.06.02 to 25 Nov 2011 but in Feb 2012 he stops having lunch with me as of 26 Dec 2013), seems more subdued and his throat is quieter.  Has had persistent AF at least since 2009. On digoxin and ankaron (Amiodarone). Also on Warfarin to INR 1.25 to 1.5 On 18 July 2010 will be hospitalized for electroconversion but first will get esoph echo to visualize left atrium and valves. He had successful electroconversion but a month or so later after treating me out to supper with red wine he slipped back into AF and has been that since as of 2010.07.02x.5.  His poor appetite before is now a bit better and throat clearing remains as it was and an interesting effect of his heart – perhaps an enlarge left atrium irritating esoph. Also we went together to Philippines 15 June to 19 June 2010 for JGC report on Mine and all went VG.  Then on 30 June 2010 his desk gets moved upstairs – a relief for me. (I was on 3rd Flr then as I am at this writing in 02 May 2016)  As of 2010.07.02.5.1705 .   On 2 and 3 Dec 2010, he had eye cataracts done and far vision. 14 Nov 2011. INR 2 to 2.5 from my asking. 25 Nov 2011 over Bami lunch (Last w me to now he had modest amount Cha Han and told me his male dog Medetai, or just Tai  is 16 y/o soon will die and almost blind and can’t hear and seems to have bad prostate and bent back but still walks him 3 or 4 x a day. The dog is what prevents Aoki fr inviting me to his house. In June 2012, went to his house for piano & violin recital of daughter and Prof Tu. To then, he was going strong but I no longer saw him much or at all and no more lunch. (The dog died)
Nov 2013, he invites me to go out evening with Ms Sally Lee (Met Sally Lee again at EAJ 3rd Floor, 2017. June promoting new company.)  and we go to Tokyo Skytree area near Asakusa and Aoki drinks a lot of wine and appetite good, so going strong.  In Spring 2014 he had herpes zoster ophthalmic branch trigeminal nerve with post zoster pain. On 22 Nov. 2014 seems OK.
But in end March 2015 he sent me email announcing sudden retirement in April and then did not answer my one email about a month or two later.  But in early 2016 he answered my postcard thanking me.  Aug. 15  2017  I have not heard from him or gotten a communication since the October 2016 last Seminar. He looked and sounded normal then. But indirectly I hear he is still OK at home and expect to see him at yearly seminar 14 Oct. 2017.  I did; he came to EAJ in evening after party. Have  photo on Desktop. So as of now, 16 Septr he is OK and I’ll see him at EAJ Seminar 26 Oct.
Update, o2 Aug 2019, Aoki came to my retirement party in July 2019. Frail but mind intact. In Oct 2019 met him at EAJ annual seminar and he seemed unchanged. As of  05 July 2020 he remains as has been.

Harriet Zimmerman June 2009 a year older than I, my then  77y/o cousin on her vertebra fracture: tripped over my 2 year old grandson who was creeping behind me on the floor and (I) fell backward, totally unprepared to break my fall, landing on my back. Since the pain was quite different from anything I had ever experienced before, I went to the ER in Danbury, CT. The x-rays showed a spinal stress fracture.  I stayed in Ct at my son's house for about 3 weeks, then my daughter came up and drove me home.  It was very painful to lie down in a bed and now after nearly 6 months it is still tricky getting out of bed in the morning and while I do get around, drive and visit and shop, I still have some back pain and discomfort.  I have lost some height and have a slight  s- curved body that I never had before.  Prior to the fall, I had never had any symptoms or any indication that I was at risk.....I have just started taking Fosamax hoping to build up my bones.  Good luck to you, Harriet. 2013/2014> Happy new year to you! Yes I am still driving, would be lost without the ability to go wherever, whenever I wanted too! My back injury has impacted walking ability and balance, so long hikes in the woods are off limits for now. Still manage to shop, engage in social activities, etc. so far so good in the overall health department! I do have to admit I could never exist on your eating pattern, still enjoy creative cooking and eating yummy food! And yes I do watch my weight and eat lots of veggies! Stay well.
Harriet is still going strong as of date, 2016.05.02. On email list.  Asked to be taken off email list later in 2016. Have not heard from her since  as of now 05 July 2020. Sent email 21 Jan. 2021.

Geneva: Geneva as you by now probably know is a terrible place
for girls. There's a legal thriving whorehouse area near the train
station but the merchandise is third world and not at all appetizing.
The good looking hookers work over the phone with help from the
concierges.  They all specialize in Arabs and they start smiling at 500.
Euros. I never wasted my time there. The best I could do was via the
free newspaper want ads and the internet. Like you, I am not a bar
seeker and only went there when I had to accompany a crazy client but

In Todai Hosp 3 days post op cataract left eye. 2011.08.19. AM pre bkfst 63.5 kg & 1.74 meter, ess. stripped for BMI 21. In Todai 2012.12.03 morning light fast in pajama 59 kg, BMI 19.48
On bisoprolol beta blocker several years. First 2.5 mg but for last year or so 5 mg a day.  June 2009 morning HR 55 bpm. SaO2 97% Lowest early March 2011 105/55 and heart rate 67 bpm  2007.12.11. Still going strong on it in 2021.
Orthostatic hypotension (2007) just after bath continued for minutes to bed (Probably arrhythmia). Has not repeated as of 2017 to date  and since that 2007 I have started beta blckr and ACE-I. Latest c. 120/70 and HR 60 to 65 at Miho by nurse.  Latest 16 Aug. 2017 04:25 EAJ 138/78 53bpm just after meds.  Insert here start Septr 2018, I increase bisoprolol from 2.5 mg to 5 mg QD and continue ACE-I 2 mg and have noted good response with BP around 120/60-70 and HR in high 50s. My Body Measurements: Waist 2012.04.02 11:45AM 14 hr fast after peeing 61 kg and 93 cm waist; previously, (In early Septr 2018, measured 90 cm waist at Yokohama store) a few hrs after big lunch 2011.11.25 it is 97 cm w deep breath 36.5” or 93 cm (On July 1 2009 a 95 cm waist just borderline tight) Latest waist 2013/12/27 at 56 kg, at 23rd hr of 24+ fast, 84 (?94 typo) cm. and same 2015.09.12  Latest Aug. 2017 93 cm tight for girdle.
In late July 2019 I ran out of beta blocker and ACE-I and after several days noted rsting heart rate 90 to 122 bpm.  I guess super sensitivity to betqa receptor. Restart beta blocker 5 mg and ACE-I captopril 2 mg and within a day bpm 66 to 80 resting.
 10/7/2004) Neck 14.5” or 37 cm. My head circumf 59 cm, beyond upper 2 SDs for 18 y/o. No change.  2 Apr 2012, going long and strong with good BP and HR on all the meds and vits and minerals. (Note addition ofchlorthiazide type diuretic c.2019.) Most worry recently is left hip (2014.06.22; turned out osteoarthritis diagnosed at Todai with x-ray this month. I hope to get hip replacement because painful and I limp now) and also the pain comes and goes ache in both upper femur/thighs (Note, first mention of the thigh and glut muscles soreness) 2012.04.02 the aches have gone and since restarting vit D and Ca the left hip upper femur that had worried me has improved 50%).  But by 12 April 2013 latest the left hip is at its worst with fairly constant rather sharp ache on standing and barely can manage to walk w/o limp. (By early 2018, almost could not walk) And has continued like that to present   But now early Aug. 2014 a bit better from corticosteroid rub in ointment courses. But by 2013.12.27 now no more worry altho same symptom. It must be osteo plus the PSA very low. It turned out to be osteo.  By 22 Nov. 2014 even better and continuing corticosteroid oint wrap once a month.
Latest 2015.09.12 Left hip pain moderate and bothersome with limp but I tolerate it because afraid to get hip replacement. Stopped the cortisone ointment last year.
2016.05.02 Tolerating left hip. Decided against replacement. (Was a bad mistake)
But by 2017 decide in favor of total replacement and starting the route first of heart check up and then of ortho surgeon consults.   Now, the very latest, 16 Aug. 2017, 04:30am, for last several months both quads very sore (Note start of that symptom that has not changed and now 02 Aug. 2019 and left hip osteo and right  waist movement pain to almost can’t walk but saved by daily ASA 500 mg  (Decided against surgery; rely on girdle and ASA and careful; again it was a bad mistake; had the left hip surgery June 4 2018 and was like night and day - so good result and such easy surgery. )

 Also my heels better April 2014 and continues tho peeling 2013/12/27 and still Ok Aug. 2014.  And by 22 Nov. 2014 after years of walking on left osteoarthritis hip, the callus of left heel definitely less than callus of right heel.  So callus amount is from pressure effect.  Same as of 12 Sept 2015.
Right sole and heel waxes and wanes mostly in winter and better in Spring/Summer. At its worst controlled by Linderon/Genta under tissue
 02 July I guess GERD sometime but overall good and minimal paras.. On 2013/12/27 GERD in hour after meals in chair only and just that bad feeling with slight ache shoulders.  By mid 2019, all callus gone. No more worry about it in 2021.
 Still bothered by GERD in Apr 2010 but no longer by April 2012).
5 week remission on feet after saran wrap corticosteroid 1 week (2011 and by Apr 2012 completely forgotten). Do daily hot bath but no other precautions. But by 2011.11.25 heels thickened and soon will need next steroid wraps – note seasonal affect – but so far in 2012 by Apr completely OK and no more steroid wrap)  2010.05.26. OK with occasional corticosteroid Saran wrap as needed. No sox policy seems good. 2010.07.02 some relapse both feet with fissures but not bad.  Still same as of late March 2011. Now 14 Nov 2011.Latest 12 Apr2013 OK. And as above now 22 Nov. 2014.  Did Saran wrap before latest Aug 2015 trip for 10 days.  Continues remission as of 2015.09.12. As noted for 2016 above. Similarly, 21 Jan. 2021.
And since then complete remission to date, 16 Septr probably due to very little walking. A bit para about prostate Ca felt small nodule – early Nov 2011 (In Feb 2012 felt it again not sure if change). Left hip OK. PSA latest 2.16 and detect multiple small nodules behind part of prostate on rectal c. Summer 2012. But by early 2103 to now 12 Apr the PSA OK no rise and sometime less than 2  As of 2015.09.12 all prostate paranoia gone.  Same for 2013/12/27. And now same 2014.06.22 and 22 Nov. 2014. Also 2016.05.02 and to this latest 2021 update.
As of 2017 PSA normal.  Latest PSA and on IM 500 mg testosterone, on 13 Septr 2018.  Now since April 2018 on monthly depo testosterone 250 mg, last shot, 21 Septr 2021.  No side effects, feeling fine. Similar early 2021.
 GERD sometime in chair only after wrong foods nut causes miserable feeling subtle in chest and worse mentally. Same to 2016.05.02
 Spa OK. Eating a lot of Cup Curry since April 2011 EQ time. But slowed/stopped by mid Nov 2011 But then continued and still as of 2012 Apr using light lo cal Cup Curry  and that continues 2013/12/27  Also my boxes of spaghetti probably from the Volvano spa below so date sometime 2013.  3 minus 2 packages cartons left.  But gve about 3 or 4 packages to Yuko.
 But rare cup curry and always light as of 2015 Nov but now 2015 sept no more Cup Curry at all for many, many months. And continues to date 16 Septr 2018.
And also z 1.7 mm spa 50 cylinders at a time at least once a day with Mitsuba (Then in 2013).  Also tons of Mitsuba for past 6 months.(Then) As of date, 2013 Apr 12, no changes other than slight weight loss. Prostate same.  Prostate para gone completely because PSA OK to 2016.05.02 and as noted 1.71 on 13 Septr 2018.
    Stop restaurants since 2014 except occasional runs.
My Breath-holding me, on 2017.03.17.5.c.0800 1 minute. (Still, 27 Jan 2021) Big decrease from last   2014.09 Aug. at desk in EAJ c.17:30 held it 2 min 30 sec and a little plus. On 2016.05.03 2 min 25 sec b-hold. On 16 Septr 2 minutes. And my VC a little past 51. 2013.04.13.5.1354 just made it past 2 min and vital capacity just past 5L still not much different, maybe a little greater on 16 Septr 2018; 4/2/2012, 2 minutes + with no preparation; on 4/10/2006, it is still 2 minutes 8) and 130 seconds with puffed cheeks both holding nose. (2010.07.02.5.1720 I test after 4+ years looking at internet digital seconds clock and with puffed cheeks and held nose and about 30 second preliminary deep rather rapid air breathing and get 140 seconds, or 2 minutes and 20 seconds – an improvement over the prev. 130 seconds but probably not significantly longer) From literature breath holding >1/2 m normal, 3/4 m occasional and 1 min rare. Experience with 4 bloggers all around 1 minute. So no change over the years except since 2014 to the 2017. But now in later 2018 on TE and with Hct up to normal after the surgery, I am back in the 2+ minute range.
My Cell phone My 2008.09.05.5 local for USA cell phone. Number 1 917 3741573 still going as of last trip mid March 2017. But ended in 2018 Summer. From then 080 5034 9898.

Yuko using old phone in 2016 05 trip. My Apple 13 is 32.5 x 22.7 x 2.41 and 2040 grams. Or 1.147 gm/cc but around Sept 2011 it was stolen from Dan’s and Sept 2011 I bought MacBook-Air 11 inch for  c.$1000. Will be my last computer. (EAJ got me a HP c. July 2014 and in mid 2015 I broke the McBook Air inadvertently falling from my travel cart.)  The MacBook Air stats are 29.7 x 19.0 x1.0 and 1075 grams. Same as of 19 Nov 2012 but lost the internet plug in on recent trip. Using Apple for USA WiFi as of Apr 2013. In 2013 they got me new Softbank plug in for Jap internet and use it on the Apple. And same as of 2014.06.2014. And note new HP July 2014. The WiFi plug in broke 2017.03 and being repaired. Also Ipad starting 2017. ,id March but I accidentally dropped it and it petered out Summer 2018.
      My endorphin Yaki Niku 9PM end yr and full stomach then endo w P early AM on futon   2008.04.03 delish wake up 1 hr post drug after usual spa & wine at night and mori soba prev day – odd day for medicines. Also after shower and water as usual c. 0600; again delish W-up 04.18 0400 after early AM Singa jerked beef, carrot and usual wine. Also ASA 100. Rainy. Laydown 0200. Endorphin feel better on back   2010.04.15.4.1203. At Dan’s I ate c 11AM 80 spas w fry in olive oil onion & tomato chop over and over that 2 radish w leaf, a little parsley all cut. Also Matzo ½ w light c. cheese. PC repose in rocking chair w ¼ P and it was delish endo. Day before same meal except added 3 Swansdown pancakes afterward and on sofa GERD very bad in 1 hr at night    2012.05.10.4.0624 to 0724 just after bath/shower/shave and office cooler water drink, at end of a night on futon fr c. 0145 and usual wine & last food c 2300 w inari 4, fried chicken leg, seaweed fr Queens, pizza slice & mitsuba, also a good day with Woody Allen/Yuko letter/Kuri blood test result and good PSA. Then took quarter penta 0624 and remarkably good full hour major endorphinization tot body。Most of it flat on back on futon in position of max relax. Even left hip affected.  Sunny morn.  No longer even/odd med sched; now QD. Right now 0755 at EAJ desk still feel it moderately. Note empty stomach too except the water just before start。
2016.05.04.3(Golden Week)2148EAJ: From this morning was OK. I went  to flat c.0630 feeling GERD, drank the water, bath and relaxed till 0901 and out to buy curry rice at Matsuya and the good chicken spa at 7/11 and had 1st curry c 10 and not bad after and then someone brought lots of food including Kamaboku cheddar cheese as sausage and I had the chicken spa with 1 kama sau and then felt good and after an hour very much felt like apple pie and green tea and had double ap and double green tea and 1 hour on four cheers and vg endorphin with pleasant full sto and still feel good sitting up. Also just firmed up reservation 13 to 17 May and will fish with Leo.
   Note: Severe attack GERD  6 PM to 9 PM, 16 July 2020 treated and confirmed at Jikei ER with PPI. EKG OK.  Troponin nl. Blood tests w CBC OK but BUN 27. Meet Dr Kei.

Also lady fr Spain (Room cleaning woman.) day starts 10. BTW, the Lady Fr Spain stopped in early Dec 2013 and they do not replace her.  But replacement comes May 2014. S tarted June 2014.  Now in Aug. once a month. Miami Beach vacation: earliest circa Dec 1940 (?) we drive and I recall pleasure of waking up in Georgia or Fla motel with smell of South – citrus from tree. Each yr go in late Dec. 1941 was probably first by train but memories unclear. 1941 PS 94 Miss Fennel 3rd grade.  Fennel memory of me on some kinda scooter coming around corner Tryon and Gun Hill and almost hit her. In 3A I was not allowed to leave room to pee by cruel Fennel; and, in class pee’d on seat because she wouldn’t allow me to go to WC.  And for years after when I went to the seat I could see the outline. I’d my dried-up pee.  Seems this is year of IQ test there.
  7 Dec 1941 I am in Wayne Ave living room lying on sofa along east wall, it is sunny day early afternoon and I am listening to football game. Also looking out fire escape window and seeing air writer mysteriously in sky. Pearl Harbor announcement on radio. c.Wartime 42-45s Fun weekends at little & big Helens. Driving home and seeing factory lit up and hearing Gabriel Heatter (1890-72) on MacArthur at Bataan  Maybe  by 1943 sex experiences with Estelle and also in my house with their visit – maybe to ’44.   1944 PS 94 6th grade to end of ’44: Hooky from school begins probably spring ’44. Important date marker because I play hooky from PS 94 one weekday and go to the Mosholu and saw it and same place was Beatrice a fellow student doing same and I felt her up and walking home told her about my masturbation. So this must have been late May or early June 1944 and helps date my hooky, my independent movie going, my blooming post puberty sexual life and masturbation.  Summer 1944 Green Mtn Camp.  Graduate PS 94 and Jan 45 start PS 80   During 1944 was much interested in my own chem. set chemistry, buying chemicals from store on Putnam Place and constructing complicated distilling apparatus like Murray Leinster’s Bud Gregory stories in TWS, not understanding principles but knowing the dinguses.   Early spring or late winter (March) 1945 – chemistry explosion in my face. Dr. Roginsky. Sulfa drug on face. Dr. Roginski was my first doc and his office on Mosholu Pkwy E. & N building just to left of E. 206th St as you face the park and on ground floor Pkwy facing apartment. He made house call on me earlier – 1944 or 43? when I had acute joint pain in ankle w fever and thought to be acute Rh F. Lastly he treated my bad sprained ankle in 1946 and I transferred to Dr Sonnenblick under Joe`s wife Bobby’s influence. Baseball, 5 July 1945 Wednes, double-header Cards vs Giants at Polo Grounds and Augie Bergamo has great hitting day with two home runs, one grand slam and total 9 hits. I go alone and sit in mezzanine behind baseball diamond. (Specific memory!) (Deciding  The Knight Jrs gang probably dates fr 1945 but might be 1944. Source of name? Smooth black and reversible. c. 1946/47? Fishing Surf casting and line casting and catching Miami Beach 1946, stingray. August 1946 book review of The Hucksters and 27 Aug 1947 release of movie. And recall the letter from Cynthia just as I am getting my University Ave Office into shape so must have been May 1970 I started the Cynthia relationship. She gets Abo July ’70 c 3 mos. Runaway, ’76 late spring –total paranoia - start once by train to Chi then fly back. A bit later fly Spring ’76 to SF for 1 month Birth Control Primer for Women idea and hysteria over 78th St.  Stay at the Lions Hotel and afterward visit Jim at C-U before coming back together. I was truly paranoid. (First noted at end Jan. 1974 right after near death syndrome.) Probably started from anoxic brain effect and continued due to the extreme stress. The paranoia gradually improved but I noted it thru the 1980s relating to my potential taxes and I sent my records crazily back and forth. Even the Ax affair partook of it. Finally, in the 1990s it dissipated though the last bit was worry over pauperism up till c. 1995. Then it disappeared, especially since the 2000 stay in Bronx although on the return in Nov I showed paranoia for the narcs lurking by my apato. Once I moved in to Coop Fuji Dec 2000 it completely went. I am now almost completely self psy (But still traces of the paranoia about money show up cf Yuko credit card) and in fact tend to be anti-paranoid.
 Feb ’75 mother dies in cardiac failure fr flu but I am really responsible because I forced her into hospital when she did not need it. Bobbie Heller offers self. Dec ’75 Xmas party decorated by Riyo at Ctr. Dec ’76 last Xmas party by Sophia at Ctr. 1977 the year of chaos w Sophia ends with her pregnant in early ’78 after Middle East New Year trip. 1978 also chaotic ends with my taking Riyo to Café Carlyle. 29 Feb ’79 after my scandal I start working Pelham Bay. Joanne Haig fr Spr to late fall ’79. Then Riyo early spring ’80 on Pelham Pkwy and summer ’80 we drive to U of I and after go to Watertown and then Riyo goes back early late summer 1980. Septr ’80 first see Zulette and winter or early spr 1980/81 Okinawa. Last see Dorrie in Hotel office winter/spring 80/81, 3 years before her death and Riyo came to Zulette Spring ‘ 81 then Summer 1981 drive to and back to U of I last see Riyo late Summer ’81 at JFK boarding gate. Then drive to U of I Champaign-Urbana till May 82. Riyo died 12 May 1982 and I go to Japan c. 10 June after moving to Mt. Sinai Chicago June ’82. First got the  left Hernia repair at Carle late June ’82.

Very slightly panicky but used technique of visualized my mom and dad’s face and repeating it’s the best possible place and it worked. Better than 5 years ago. Will see Katsu 16 July 0900. One more para by the board. (EEG March 2013 suggests rt focus and next MRI brain at Jikei will be Spring 2013 but it actually was in Nov. 2014 using unenclosed gurney and no need for injection. Will discuss result with Kuriyama week of 1 December 2014) But first I saw Dr Shimizu in Fall 2013 and he told me EEG OK and put me thru neuro exam. Then I decide to cancel latest MRI and ill see him in Jan 2014 and get EEG march 2014.

Early 2011 without intending to I go off coffee in favor of the green tea bag tea at EAJ. But I still use coffee for particular need in early AM.  Still  off as of Dec 2013, 08 Sept 2015 using coffee mornings only from machine downstairs 140 Yen when they do not being coffee bag to office.  Same to Sept 2015.  In 2016 I use coffee when I get free powder. Now on Yuko BF Brazil coffee. Update 27 September 2021; on controlled coffee intake as desired.
Similarly 22 Nov. 2014 but several weeks a bit more coffee from machine in AM for writing stimulus.  By 2021 have 1 coffee mornings only at SAN Marcos Cafe. But stopped that early on!

 2011 Summer I get idea to rent a Homat in Roppongi and google it and one available. The idea is to live with Olga and kids (my desperation). I ask the housekeeper to go with me to look at it. (Actually not related; he was switching jobs to Kawasaki.) Despite no guarantor I could get it but come to my senses. Also Aoki helps by refusing to be guarantor. Summarize fr Dec 2012 to now 28 Dec 2014. In early Dec 2012 had my left cornea DSAEK and success. (See DSAEK later under Amano.)
 Meanwhile Yuko starts work for me in Dec 2013 and to date great success –(Ended July 2014 with my need for sex messaging but resumed in Sept 2014 after she comes back fr Honolulu and going good now to 12 Sept. 2015 and further to April 2016 when she starts several week stay in NY area.)  4 hours each Sat for Y10,000 per session. But actually usually only need for an hour or so. She copies from Lib at start but not later, and she is now into editing and proofing.  Since Sept 2014 restarting she comes to EAJ and sits by me. She has continued except month of Aug. 2018, here Nice France home stay and we had a falling out which I ended by admitting I’m wrong and agreeing to pay her upped rent to 85,000 fr Oct 15, 2018. These last years she is mainly updating and typing Argosy stories into the website A of K. And she works till 31 Dec. 2018 when I end EAJ. Then helps me until end feb 2019 when she goes to NYC.  So overall 6 years from 2013 to 2019. Now, Summer 2021, she is completing 2-mos in LA for marketing employer and plans 1-yr journalism MA course in London from Aug. 2022, and I approve!
   I continue in pretty good health. But in late September 2021 xrays and CT suggest malignant lower vertebrae possibly multiple myeloma!
 Steve’s CLL started knowledge of Fall 2008 and summarized below later. My eyes stable since last op 2012.   Last exam mid Sept. 2018, stable.
March 2014 – DVT left calf and moderate bilat ankle edema for a time .  Then stabilized and since April/May 2014 noted band hypalgesia ankles area.  My guess from combination weight loss and too much sitting. Anyway seems stable by now and in good spirits and health now. In Aug 2015 Dr Ropper checked and reassures me.  Follow up as of April. 2016 the band at a little above ankle level remains, all edema gone, all calf problem of hardness gone. I do much more supine recline in chairs at EAJ and more time supine at the flat. The BNP was up and then coming down but last one still high.  Briefly was on Coumadin to test out INR and found one should start with 5 pills at once. Now long stopped. Upped aspirin to 1 pill 325 mg a day.  Essentially back to square 1 except for the paresthetic bands both feel up to just above ankle which are stable and do not involve mjuscle (But by 2017 my ant thigh and glutei sore and weakened and strophic and continue stably to date latest 2018.  (And June 24 2020) or any other sensory modality. Same to 2016. They are due to subQ edema and not neurological.

Bobby , my sis in law nearing death; She died December 2014. Has stopped chemo for her bone cancer. 22 Nov. 2014 now she is on terminal hospice care according to Rich. Died Dec end.
Jim visited NYC Steve and I hear Jim had prostate CA and radical prostatectomy.  Also head trauma fall ice skating while on daily ASA and SDH and craniotomy drainage.  Now seems OK and we will meet sometime later this year, I guess. (We did not; he visited Tokyo 2017 and 2020) But as of 24 Nov. 2014 he ignored my email a month to 6 weeks ago but only responded to my query “Are you OK?” and said he would get back to me but he has not. (Now he has Septr 2021; all OK.)  He got back to me but did not respond to multiple requests to meet.    As of mid March 2017 on verge f visit to Japan with Koko he emails me and all seems OK. (He visited in 2018.)
Koko nice visit to Tokyo as noted. Now new job near Las Vegas.  Latest by 2017 moved to Henderson NV with good promotion at Charles Schwalb and new flat fr Septr that she invites me to have a room when I am not in Japan and I went and stayed 3 nice days and AOK for future.
More on Yuko. I got her to see my dentist and she has many cavities he will work on. She saw Amano 25 Nov 2014 at Inoue Clinic for complete myopia check and all OK  She is signed up for driving lessons at the school that also includes CPR training and lessons start in December.  She flunked and gave up driving, and CPR will be in June 2016. She saw Kuriyama at Miho and he drew bloods and result OK.  She got my advice to only apply to EAJ from March 2015 as part of Senior year applications but she prefers not to and graduated April 2016 and now on NYC area stay. But came back after few weeks and with me since.
Also I gave Kuriyama 200,000 Yen gift preliminary to my Jikei Hospitalization.  Also Yuko and I are sending El Sevier the correction on Fundamental Neuroscience 4th ed 2012 to ms Mica ala Dr Larry Squire’s OK.  Done.  Then to now 05 04 2016 all basically OK and I  have made very good trip to AAN in Vancouver free as student in April and will make trip to NYC for Alex wedding in mid May 2016. Did it.  Next month to AAN in Boston 2017.

Erma last seen well at her 02 October 2005 (BD 02 Oct ’22) birthday celeb. Then her senile dementia (Same as her mom) already noted got worse and she needed nursing home and got pneumonia complication. Saw her in Hospital October 2007. She died 2008.04.28.5 age 85 after chronic pneumonia. Note the c.2 year lag time for dementia to put her in nursing home and then c. 6-month lag time that could have been much shortened by euthanasia. Steve moved from LI  to the Gatsby Gardens in Summer 2013 and then, by Oct. 2014, was at 65 Bank St,  bus nr 977 7950/Fax -7762. Mobile 1 917232 7382. Then last yr, 2017 he moved to current 56 W. 76th St Penthouse (Brownstone-like) v4F and I’ll see him, 18 Sept 2018. I saw him the evening and then walked out at c 5 am and he seems pissed forever at me and took me off his list.  But now in Aug. 2019 he is living on the North Shore LI near Oyster Bay again and I shall visit first weekend in October and we are OK again.  By 2021 he is again in Manhattan.

  Joseph Joe born 6 March 1924, died 3 Nov 2003 1107 (AM, Hawaii Time) at Lihue, Kawaii Maui Hawaii Isle of drowning due to rogue wave with BJ and Bobby in water but escaping.. The death certificate says other condition “severe coronary artery atherosclerosis”, “cardiomegaly”. (Probably autopsy finding but not stated) Rich says only Bobby and BJ were present. Certificate says “found unresponsive in water at (Paipu) beach”. Bobby died end Dec 2014.  Bobby Stim's date of birth: 10-06-26

  Steve Stim, CLL, see other entry. As of Septr 2021 going strong with same partial remission and WBC up to 90,000. He had nice visit to Tokyo with Alec and they came to my office and I did my spiel and satisfied about that.   Will report this visit 9/18/2018; still seems to be going strong but high WBCt. Still is as of 2021.
 As of 2 April, 2014 going strong in partial remission with 30,000 WBC. 2010.07.02, have not heard fr Steve 3 -4 mos. Now 2011.11.29. Still going strong in remission. And see my video made Jan 2012 that summarizes with Steve his CLL to date. As of 8 Dec 2012 going strong. But in 2013.04 last WBC 120,000 but no symptoms and Hgb OK and doing everything and jetting.  My contact with Steve by 24 Nov. 2014 and have seen him since – and see above) At that time his WBC c.60,000 and still OK in all ways. No new chemo. He told me he planned to move family from Gatsby’s to NYC (Must be 2014 because by Oct. he was at Bank St)  and I said I’d pay rent out at LI if he needed it and he cut contact shortly after that as of date now 2013/12/
Steve going strong as of March 2017 and see him 9/18/2018. Saw him all evening with Adam, Alec, Doug, Claudine and Sandy at W. 76ath brownstone type walk-up 4th floor and slept in side room and left 5 am unannounced and pissed him off no end. But I visit him in new place on North Shore in spring 2019.

Playing hooky, caught perch and sunnies and on a weekend hooked from behind small bass my mom cooked for me with spaghetti c. 1945 at Sawmill. Sea fishing for flounder on the Olga c. 1945-46. City Island in boat with Zip spring ’45 doing hooky and bring the flounder home to bathtub and mom cooks it for us w spa. Fluke in summer 1948 or so off Sandy Hook. Also mackerel and bonito from
Sheepshead Bay and one time went in blizzard for cod 1948. Also with Dad on party boat and he got terribly seasick. Casting fr Miami Beach beach near t Delano maybe 1947, catch fish w older man watching; also catching the ray. In Summer 1964 NH Pickerel in lake with lure then in Summer 1969 in New Hampshire – trout caught in river deep pools. In 1964 or near, we Bronx Hospital docs chartered a boat fr Montauk and I snagged shark by tail and  that broke my rod and then helped pull in harpooned sword fish. Ate it for weeks. And eel fr Long Beach jetty w Riyo in late 60s. Cat fish fr Sawmill river w Jim c. 1960s and also bass fr Botanical Park Lake and later many sunnies fr there and VC lake. Fishing w Masaki Apr 2007 – I hook 1 flounder but it comes loose.  Bluefish c 5 caught Island Current out of CI October 2008 9/14/2000 late in day caught c. 10 sunnies and re-placed them in my personal pond but later seemed not to have taken but never sure. Sunnies, a few, Summer 2006 at VC. April 2007 filet and cook out Masaki’s flounder in the glen. Fishing at VC c.2008.09.15 over 2 days catch 2 small mouth baby bass and several sunnies all on worm with line. (Now, 2 April 2010, visited 23 March and saw angler catch a sunnie on worm and casting. But no bites myself on hand line and water temp 60F on warm 2PM. The week after it was down to 54F on freezy cold Sunday. Late/summer-fall Sept 2009 caught 3 or 4 over couple days time. That summer kept two in aquarium at Dan’s. Got to watch because they jump. Then brought 2 to Steve’s pond but eventually they died in late fall. (See more notes in my DJ on other flash clip) Writing this in 2012.04.02 caught biggest sunnie ever in latest 9/2011 on worm in AM and brought home to Dan’s with on the way tripping and going into Montefiore to get water for it but after living a day it dies in tank and I eat. See more in pix too. Since then been out on Island Current c. October 2014 (Steve was at Bank St then) and caught 2 sea bass and 1 porgy but had to throw away because Steve finked out on me. Also that time in 2014 noticed that due to construction about the City Island Bridge my off the bridge side fishing spot from childhood is now extinct. Latest Island Current 2016 Porgy with Leonardo Went up there morning 2017 with Yuko but too bad weather My Fishing wisdom: bergalls lived about 1 1/2 hrs in jar filled w seawater...; late 8/99 five sunfish at VC lake and cooked-out. Sunnies nicely cooked by just throwing them on fire and retrieving and then you pick off the flesh fr the bones. Snapper fishing CI 8/31/99 caught 2 in peak season for surf casts. Sunfish caught 6 at VC lake on one worm, Sunfish or Pumpkinsed or Lepomis gibbosus. It can take temp range 4 to 22C or 39 to 72F. Deposits eggs on gravel bottoms late spring. The male has red spot on operculum. During spawning the female has swollen belly and you may express ova. Male has less obvious yellow bands on belly. The adult fish may be 15 cm (6 inches) or more. Mating and spawning may go on May to August. Male brighter blue in head region. Stripes brighter. Male head slopes more. Female head less elevated. Also I noted in my tank that male seems to pursue female.  Also the fish love to eat live guppies in the tank. Latest trip Aug 2015 saw 1 foot bass caught a VC lake.

Tanuki soba seems to make feel high good in afternoon. I take it on day when for one reason or other I miss usual breakfast at home.  Also only at the near subway soba place.

(Not really; still once a week as of Nov 2011 but since stop) Also note corn very bad for my GERD   When bkfst for treat the soba doko tanuki soba c. \500  By 2008.08 Aoki seems to have given up lunches (Temporary) and I am down to maybe once a week w Ishioka and doing more McD 100\ Hb lunches at EAJ. 2008.09.03.3.c1300 near station west side Hakusan Dori Udon place, Aoki intro’s me to \290 oyako beef donburi – Yummy tummy.  Curry always good. Once we went to the noodle place across from    Latest lunch entry 2012.04.02 Now since Jan 2012 no lunches outside except rarely with Ii san at Bamiyan when I need him for paper work. Continues into present Nov. 2014. And by Sept. 2015, absolutely no outside lunches in 2015.
Lawson pretty much less freq. from Oct 2012 due to eating more from green grocer & Queens and use of light cup curry and my spaghetti. It means less calories and weight below 60 kg. but still crazy eating spa w meat sauce and spa with meat burger and ffs and salad, or sometime curry katsu instead of spa w meat sauce – this for all 2013.  Now in Nov. 2014.  Continues to improve during 2015 now completely Queens, green grocer, my spaghetti. Occasional McDonald Hamb.

My eating has educative implications for those interested in living longest.
First the idea that one's eating behavior constantly evolves. Most of you dolts are critters of routine and habit - you are eating daily a constant menu that varies predictably and that will put you into an earlier grave than if you follow Physician's Notebooks and also that gives you a less happy life than I who follow PN have.
  My over-riding health principle in eating is that quantity trumps quality. Eating less is more important than eating "healthy".  Do not take that to mean that high quality (in scientific health terms) is not important. It means it is not as important. So do not let it out of your awareness while you are eating.
The eating that gives longest, youngest-appearing life is relatively low calories (for a human 1500 to 2000 calories a day, or a too-low-calorie-to-gain-weight diet) with adequate vitamins and needed minerals. This was first done experimentally in rat colonies and has been confirmed with human volunteers. It adds 25% length to life expectancy and not only that, but the aging is prevented (the old rats are "young" physiologically) so that actual aging is truncated and like the grandfather clock everything stops suddenly after running perfectly.
But you also have to consider happiness. You cannot live well in a semi famine state. But you should always have what I call an "edge" (a special feeling to achieve a particular aim", a feeling that goes against everything you were raised to believe in) in favor of low-calorie eating.
A  good point of my "philosophy of eating" is that I can enjoy a lot of relatively not so healthy food but make up for it by an overall low-calorie-per-average-24-hours eating. That philosophy allows me to "live off the land", meaning, adapt my eating behavior to where I am presently at and the conditions thereof.  Always I have the over-arching aim of long life, good health, but also a closely secondary aim of "Enjoy! Enjoy!" Also I am constantly scanning and monitoring my immediate after-eating response to a meal. No matter how healthy the meal seems based on book l'arnin', afterwards, if it make me sluggish, or aches my belly, or makes me otherwise unhappy, I look for the culprit in the just-eaten food and eliminate it from my future eating. So I want a meal that is healthful by Physician’s Notebooks' standards, that makes me effective and energetic not long after, and that gives me the most joy as and after I eat. (I am always reading while I eat - I do not listen to radio or watch TV while I eat because it diverts attention from the pleasure and does not give me anything good)
  Consider my old cousin Dan. For his last several years at his house he got his daily food from an outfit called "Meals on Wheels" which is a weekday daily delivery of TV type dinners that he paid $1.20 a piece for ($6 a 5-day week). Dan's eating philosophy was different from mine: it was quality (from his ideas) over quantity. So he avoided foods he thought harmed him.
  It makes life very, very, very interesting and enjoyable, and also makes for a greatly economic eating experience. And, to boot, it is giving me an enjoyable life.

  My diverticulosis 1st discovered at Jikei c. late 1980s on failed colonoscopy and f-up BE about 10 or more sigmoid. Never a prob but alert to risk of hem. As of now 2018 no probs and I take ASA 325 mg every day.     My GERD dates fr late 1980s and peak worries 1990s. Scopy at Tulane 30 April 1999 showed 2 cm hiatal hernia, distal esophagitis w junction displaced 39 to 41 cm above diaphragm and on biopsy a mild sq epithelial hyperplasia and chronic esophagitis. Superficial erosion distal to duodenal bulb. Culture for H. pylori neg. After, Dr. Orlando put me on Ome and problem ended. My last scopies c. 2005 at Gan Ctr and nl. Still was worried as of Sept 2008 when I lie down after big meal and get up later with the feeling. Sometime coughing.  Going to EAJ after eats and doing drug hour in chair.   I still get GERD but only in chair in the hour after meal at EAJ and this thru 2015. It does not occur if I lie down. Has not bothered me now by Sept 2018 for last 3 years. Echocardiogram at Saisei May 2003 showed mild mitral regurg, mild ant hypokinesia near apex and mild aortic ascending dilation. EF OK. Repeated 2008.07.15.2.1400.Saisei and no change. Relieves a para. In March 2013 repeat again and no change and exact results entered in My
Will get one Dec. 2014 during Jikei hospitalization. Got it and no change.
Repeated Echo last early March 2017, EF 60%, essentially no change in mild regurgitations. No hypokinesia
  My headaches right sided related to side of head in bed 2006.04.14; 2008.04.22 rt side, but not much. 05.14 mostly occiput rt and I go to Katsu for MRI 06.27 (Today done and 16 July result fr Katsu. It was completely normal told me and shown 2009. 07.16.3) Next repeat 2013 but cancelled. And was done Oct. 2014. MRA 2016 OK.
My heel and sole callus: First caught my attention c. 1996 near start of regular returns to NYC and put down to my heavy walking. No cracks then just callus. Next is c. 2001 in Coop Fuji, I begin to note peeling and sometimes I tear at them and get sore. In c. 2003 I consult Katsu at Gan Ctr and see the derm and he thinks it from fungus and Rx’s the white cream I think the cream is the same as present ureaparu in white jar w blue screw top. I used it irregularly and not much help but the cracks appear it seems seasonally worst in winters and get in way of walking due to soreness and worry me for future especially as potential decubiti. In Summer 2004 on verge of 1 year Bronx plan ask Kuri to Rx ureaparu and he does 2 or 3 of the blue tubes. Take them to Bronx and give one to Dan and use another irregularly. By end of the year in Bronx, summer/fall 2005 the seasonal effect continues and I actually buy cheap sandals for more comfort. Then, back in Tokyo/Yoko in 2005 I have one real bad day where I almost can’t walk coming home to Coop Fuji from EAJ and at CF 811 I take bath and apply band aids and this is point of discovery of good symptomatic effect of band aids on sore cracks. 2006.10.23 starts acting up for the fall season and still with good times during late spr summer but today even in June acts up and band aid relieves. Then next key point is 2008.08.14. It is a bad day for crack soreness and, reviewing and observing, it is clear that the condition has gradually worsened, involving both heels in areas posteriorly and not on sole and both sides of distal Achilles with multiple linear cracks, many quite sore. Reviewing historically, it also involves the sole area over the 1st metatarsal-phalangeal joint – in the creases. And sometime the sole on the instep but not at this point in 2008. Something needs to be done and changed. On 08.15 I go to CF 811 and get old not used up jar of the Gan Center Ureaparu and next early AM, 08.16 start q24hr application right after sitting in hot bath of, first the steroid ointment from Kuri and, after 15 minutes, the ureaparu heavily smeared on. After five days, worried a little that steroid might help infectious process I discontinue it and continue the daily ureaparu after bath. At that point it seems improved but not definitely. On 08.29, it is clear I am in definite remission. All soreness and pain or ache gone and the callus area is getting less and less and now limited to patches some of which are dryly peeling. I plan to continue the Q24 hr until I leave for Bronx 09.08.  2008.09.03.3.1620 I continue ureaparu and restart steroid ointment for few days. Looks like a VG for my Remedies chapter. Have appt to see Pod, Ortho and Derm at U of NC 2008.10.23 w Dr. Orlando. Fr late Apr 2009 (?) took 30 days 125 mf bid Lamisil but it really did nothing. Condition stable but all there and acts up a little June 2009 in wet weather. 29 June Ureaparu and flucort and good 2 days so far. 2009.08 Completely cured. The Lamasil worked. (I have much more on this in other place.  Now 2010.05.26, I have solved the problem and am in long nice remission after using Saran Wrap cortico steroid RX) Also no sox policy now several mos. But now 2010.07.02 acting up again past few weeks – heel and even crease big toe. Using band aids. Have done steroid oint and it works temporarily but do not want to use too much. Also sometime antibiotic – now Amoxi too. Update 2010.12.05 all is stable and I am happy with occasional 5 or so day course of steroid oint with Saran Wrap (The idea of the Saran from Dr Peter Hirschorn Podiatrist on 210th St Bronx) cover. No need for antibiotics or other. Usually no socks in day and blue cloth Chinese shoes.  Same Mar 2011. Lamisil 1 month again.  Now in 17 Sept 2018 it has been gone for at least 3 years with my less walking due to hip and back problems.
Last Lamisil Feb/March 2017.
Similar 25 Nov. 2014 latest and also note that left heel significantly less callus and peeling than rght heel and I put that down to last year’s walking carefully because of left hip osteoarthritic pains.
Durng 2015 remission continues. At Jikei test for fungus was negative so no more Lamisil.
Before August 2015 trip. I did 10 day steroid/antibiotic ointment and 10% salicylic acid treatment Also nightly hot baths and no socks and Chinese shoes.
By 2016, I note do need bandaid Linderon but get just as good by using a couple of tissues over Linderon oint with sock to hold it for a day.  Now, 2021. in remission.

My Hernia op: First hernia c. June 1982 Champaign then note return c. late 1990s and bilat but mostly left. Op 2001.01.17 (Will be 18 years in 2019 and that would have been c. 18 years since 1983) Tulane Dr. Bernard M. Jaffe under general following failed caudal. On the left side, the defect was closed w #2-0 silk sutures. It was a direct. Marlex mesh reconstructed floor of canal, it was sutured medially and the suturing included the shelving portion of Poupart’s, the periosteum of pubic tubercle and t conjoined tendon. Mesh fastened to Poupart’s for 2 cm beyond the internal ring. The conjoined tendon was sutured to the mesh w #0 Ethibond suture. On right side a mesh also used for the reconstruction but no direct sac found and no indirect but rather a weakness in portion of the floor of the canal. Discharged 20 Jan. UGI scopy done 18 Jan and nl. Testes coolth since the op, still noted 7/6/01. No longer noted by 2003.2.17 and had not been for more than a year. By 2005 completely forgotten and VG result so far and no effect on orgasm. Still going long and strong now 10 Aug. 2014. And by 15 Sept. 2015 and no longer push for stool but use digital removal. Still OK 2016.05.04.  And now by 08 Octr. 2018.

A mild brain atrophy was noted in the parietal lobe of the brain. However, atrophy of the hippocampus and para-hippocampal gyrus was very mild. The degree of the brain atrophy was comparable with that of the previous one (2014/10/20).
There was a mild intensified high signal in the white matter. Also, there was a lesion which suspect the skin defect. In addition, post catalacta operation was present.
MRA shows poor visualization of left MCA (M1 peripheral to M2 proximal), but the peripheral vessels are well seen. Chronic sinusitis was present.
Final diagnosis: There was no abnormal atrophy in both hippocampus and parahippocampal gyrus.

Lumber bone MRI early 2017
Left-side deviated scoliosis is present. There are compression fractures in the L4 and L5 levels, with the irregular bone edge. Whole lumber bone shows severe deformity. At T1W,T2WI, there is a strong signal enhancing, especially in the lower part of L4 and upper part of L5, suggesting a Modic type II deformity. There is also Modic type I deformity in the upper part of L1 and L2. The whole lumber bones shows severe stenosis, creating the stenotic intervertebral foramen (especially at Th12/L1 and L1/2-L3/4 on the right side, and L4/5 and L5/S1 on the left side). These changes are similar to that in the previous study (in 2014, Dec 8). At the L3/4 level, there are perineural cysts near the intervertebral foramen. Likewise, other perineural cyst is present at the sacral bone level. Both renal cysts are also seen.

Dear Ed,
It is a controversial topic, but I have been disinclined to attribute balance problems and paresthesias to lumbar stenosis, especially without root pain (sciatica) or claudication pain. There is no doubt the report suggests the problem is advanced from a bony structural point of view.  I can’t find the EMG in the record. The sensory nerve conduction amplitudes might settle if the problem is pre- or post-ganglionic—i.e., root or nerve and address the issue of lumbar stenosis as a cause directly.

As of March 2017, it is clear that my anterior hip discomfort is from Lumbosacral collapse and pull on femoral nerve branches.  Separate pain for  left hip weight bearing and walking from starting worse. Decision to try for left hip replacement made in early 2017.  Also noted the CT report of lumbar stenosis which probably is connected with the sensory syndrome.

My URI 2006.12.22/23/24 (12.5 flu shot) Another 2008.09.15 at 8B. Reactive cough still to 10.07. Dextromethorp immed response. Another URI noted right after Philippine trip return. 2010.06.20 and after sev days gone. And next URI on Bronx trip Oct 29 or so 2012 only lasted 2 week symptom. Severe URI (2016?) with very frightening constant cough in lie down position lasted about 3 weeks and Kuri seemed to cure it but maybe just TofT using the floxacin. Later I find Allergan is good cough suppressant.

Flu shot rt arm 2015.11.04.3.c.1600  After seemed to have increased appetite for a day. On 11.08 had ataxic getting up but maybe related to taking Loxonin. But I took Loxonin much after that and never a problem.
My Low Back For the year since 2006 have been having fluctuating sharp pains on weight bearing left hip and left low back. X-rays showed no hip pathol  but degenerating lumbosacral spine (2006 but by 2014 left hip osteoarthritis) so I guess it is nerve root pressure from collapsing disc. Comes and goes and worse after active walking day. A weekend ago was so bad I limped but gets besser quickly so I just watch it. Hot baths help. 2010.07.02. Update – do not bother about it much last months 2010.12.5. Same mar 2011. By 2013 Apr now it seems definitely worse; always somewhat sharp in left hip lat area on getting up and on walking. Causing limp almost but I compensate and do not show it.  Good mobility but sometime need to help with hand. Prostate PSA remains lo around 2.0U. Still same 2013/12/29. And stable as of Sept. 2015 and now May 2016.

My Ideas: Aging: as you pass 60s a death sentence that gets less and less latent – ups one’s daring for life by willing to accept higher risk life decisions as one ages; ending life in healthiest climate Hawaii-like but less populated. My theory of why old person gets more conservative: It is suggested by my observation of pal Lou whom I reconnected with at his age 76. As a kid he did not seem highly intelligent because did not do well in school and dropped out of HS and joined the army at 17. But when he got back in touch by email he seemed much more intelligent in that he wrote a good letter and his syntax and language excellent. Now his medical story is interesting. He got old-age diabetes and was overweight with high cholesterol having retired to home in Eugene Oregon in late 1970s after almost 30 yrs Army career ending a staff sergeant. He had heart attack and stroke and about 90% blind from stroke and diabetes. But he finally followed good advice and lost much weight by diet, lowered cholesterol and treated DM with insulin. Big effect when he went almost blind he lost reading. He was not a big reader but now he could not read at all. But thru the VA in the 1990s he got outfitted with special computer that converts writing screen to voice and also has thesaurus, encyclopedia and dictionary with word spell and built-in grammar. So he started to read again and, especially he was almost a semi invalid at home all the time he began to read much and got smarter as I know him now. So that is interesting – computers and IQ.  But more. As result of stroke he has partly defective short-term memory (It is like RAM on computer). So he has excellent memories up to few years ago but he easily has mind slips of things you might tell him yesterday. This is typical of stroke memory loss due to arteriosclerosis in brain. And as result he remains erratically conservative. For example he dislikes Obama on racist reason (tho he denies that) and loves McCain because he identifies as former military careerist w. McC’s, as older age guy and survivor of melanoma i/d’s with McC.  So I kept trying to convince him in favor of OB and contra McC. Finally he seemed to come around on McC’s VP choice Gov. Sarah Palin and wrote me an email citing her stupid entry in a high school year book and as example of McC’s bad judgment. I was gratified but said to myself: Wait, he’ll forget today’s email by tomorrow and slip back into his racist anti-OB mode. And he did. And it struck me that this is the explanation for the common observation that most older persons seem more conservative than younger persons. It is due to their having small strokes and losing RAM and so having same phenomenon – not being able to retain new knowledge and as this accumulates with aging they cannot learn new things and fall back on their old more conservative, often racist mold.

  My Cataract of eye & other, 1st told of 2000.5.16.  Previous 4/99 exam it not mentioned.Then vision exam autumn 2006, no change. In morning esp note slight haziness (First effect of cataracts – this seems to be 2008). July 2009. All stable. New progressive glasses but Rx about same.  (2 Apr 2010, no more Rx specs, and almost do not use. Stable vision. Depend on $1.49 reading glass and good light and continue 2010.05.26) As of 2010.07.02 am studying cataract op in earnest but still year or so down the road. Update 2010.12.06, I have appointment with cataract surgeon Jeffrey Schultz at E 71st St office via Dr Robert M. Schwarcz’s rec’.  Decided against op at this time as of Mar 2011 and cancelled appointment.   Using the textbook I wrote to several selected cataract experts and below are letters reply I got
: Dear Dr. Stim, (Received gmail c 3AM, 2011.04.15.5 and replied) Thank you for your letter regarding cataract surgery. Although the epiretinal membrane may not be causing distortion of your vision, it can cause macular edema, particularly after cataract surgery.  Because of this condition, your desire to have good distance and intermediate vision, and you don't mind wearing reading glasses, I would recommend either the Crystalens AO for both eyes or slight monovision with standard monofocal IOLs in each eye with your dominant eye set for 0 (plano) and your nondominant eye set for about -1.25D (if possible this should be tried first with contact lenses to see if you like it). There are excellent cataract surgeons in Japan. Dr. Takayuki Akahoshi at the Hospital is considered one of the best in the world.   Dr Stim I am not sure how you got my postal address, but I am happy to offer some advice. The most important thing about getting a good refractive result from cataract surgery is to concentrate on the surgeon, not on whether the IOL has 0.25D increments. It is the accuracy of biometry and consistency in the final position of the IOL which is important, and given that most surgeons have an accuracy way below 0.25D, IOLs with powers in 0.5D increments are sufficient. Warren Hill (www.doctor-hill.com ) has shown that the most important factor in determining accuracy of refractive outcome following cataract surgery is size, shape and centration of the capsulorhexis. Dr Hill is THE world authority on biometry and getting an accurate refractive outcome from cataract surgery. To this end you are fortunate that there are now (over the past year or 18 months) available several lasers which appear to be able to produce consistent and accurate incisions and capsulorhexis. Theoretically this should give much better and more consistent refractive outcomes from surgery. You can see surgery being performed ‘live’ at the latest meeting of the American cataract Society meeting in San Diego last month at http://eyetube.net/series/alconlivesurgery/2011sandiego/hasow/medium/ I would choose an Acrysof IQ lens. I would choose a surgeon who has access to laser assisted cataract surgery. DMEK is an ultra-thin graft consisting of only endothelium and attached basement membrane (Descemet's membrane). DMEK often provides 1 to 2 lines better vision than DSAEK.  Dr. Price frequently combines cataract surgery with endothelial keratoplasty when both are needed. He also performs both DSAEK and DMEK. For DSAEK patients should plan to come for one week for the evaluation, surgery and postoperative evaluation. For DMEK, patients need to be able to stay longer because about half the time, additional air needs to be added to the eye within the first couple of weeks to ensure that the edges of the DMEK graft are securely sealed down.   I've attached a copy of an article we published on our initial DMEK cases. Dr. Price has now performed over 300 DMEK cases, and the early challenges with donor preparation and primary graft failure are essentially solved. I wrote Dr Amano on 24 May 2011 and got immediate response. After seeing him at Todai where he informed me I had 50% (Dr Akahori said 100%) possibility of post cataract surgery corneal edema, which could then be reversed by DSAEK surgery, I eventually decided to enter Todai Hospital 15 Aug 2011 and had left eye cataract removal and IOL 16 August with 50 cm focal distance IOL. After 24 hours bandage removal good vision but by next day it clouded and since then have had almost same degree misty vision.  Left eye acuity (My mistaken observation; it is actually the reverse) better than right and it has not hobbled me visually. Instead of eyeglasses I depend a lot on loupe for reading.  Difference in cataract eye is about same mistiness but colors differ. Whites are sharper in left eye with IOL. And 10% magnification from IOL. (This all must have been written after the successful Left side DSAEK in June and after the 2 months for its corneal edema clearing effect)    They put me on list for DSAEK and in 16 Nov I went in to hospital after a call and was scheduled to have DSAEK but at last minute was cancelled because of some problem with the donor cells just at go to OR time.   Then I am called again, c10:15AM, 2012, 28 March Wed by Dr Yamagami and told they can do DSAEK tomorrow and come to Todai 10AM.  I am very happy but only 15 mins or so later he calls again and says Dr Amano has determined that the donor cells are not right for me so cancel and I remain top of list.    Meanwhile I have been putting a drop of steroid and a drop of antibiotic in left eye twice a day since Aug 2011 to present and continue and no problems from that. The actual doctor instructions were 4x a day but I modified that. (Entry 2012, 28 March 11:17AM) Continue: Finally had the left side DSAEK June 2012 and another bubble injected 36 hours later.  After 3 months good result and even that minor with loupe. By Oct. 2012 the obvious difference and increase in mistiness right eye make me ask for right cataract removal and it was done 4 Dec 2012 with IOL and I developed same cornea edema as I had before on left. Initial 24 hour and then mistiness. Surgery 9:15 to 10AM and I ate bkfst right away and walked too much so eye was leaking by next day and I got in bed and lay looking up for 15 hrs and OK. Discharged 6 Dec. and cataract surgery OK but same cornea edema.  After this surgery Dr Amano did not tell me I was on the list for DSAEK for my right side – it was too early after the cataract op to say – but I got a call from Yamagami the next to last weekend December 2012 and went in Monday and had the DSAEK right side Tuesday but because of a nurse’s error that I should have reported before the op – She neglected to connect my IV containing the pre-op carbonic anhydrase inhibitor Diamox to my IV catheter so I did not get it in the hour before surgery and the surgeon did not know it and as result had to hi intraocular pressure and he had a very difficult prolonged surgery - 4 months later I had no relief of the right cornea edema and he said so and put me on schedule for another DSAEK this time under general anesthesia .  Put on schedule 11 April 2013 for final DSAEK.  Finally June 2013 repeat rt DSAEK under general and now success. It took only a short number of weeks to see   And continues good result 01 Oct. 2015 and to May 2016 now.

(About contact lens) In Dec 2012  I am discharged with soft contact that I wore 5 weeks with no probs and I do eye drops right eye – kurabito and linderon 4x a day and bronek 2x a day and maybe first improvement today, 2013,01.25 though still cornea edema. The Contact removed after 2 months. A suture unraveling and remove March 2013.   By 2013.04.11.4. visit no improvement op judged failure and I am put on list for another cornea transplant this time will be under general anesth – he says because it lowers intra op intraocular pressure. Hooray!     (This last para a repeat of higher entry)
Now, 10 )ctr 2018 last eye exam, Septr 2018 stable since surgeries.
  My brief tic douloureux 2006.08.22, probably from tooth irritation. Did not recur. My camera wisdoms (fr throwaway in Colorado) 1. Too many; 2. Buy only one at a time; 3. Take close up human view; 4. Cooking in fry pan good; 5. Do not repeat views; 6. No far off human views; 7. Again be chary with shots, plan each carefully, stay close and one subject. (But all cameras now obsoleted by cell phone) My Video Cam Wisdoms: Speak clearly, not too low pitch and slowly and keep in mind that mic is front of cam under lens. In playback mode a lower volume is clearer than highest volume.   Note best movies were taken from stationary point like train window coming in from Narita to Yokohama. When you hold cam it is too jumpy so best to film from tripod or other stationary view if you can. The VR mode is best when you use DRW because one can edit and reuse after initializing. Edit in the head before shooting scenes and while shooting each. When you are shooting a lecture, be sure to have long enough (or two discs) time. Keep t cam stationary and vary from close up to far view. Take note of voice address system and try to be near its source or if lecturer is depending on his own voice volume sit up close. (I get FLIP from Rich Oct 2010 and first use in Dec 2010 but I find it not useful to now 11 Dec. 2012 and continue not 01 Oct. 2015) Since Sept 2013 no more vdeo cam; left at Rich’s and if I need video use cell phone.

Thanks much, Boris.  I recalled that back in 2000 when I was in the Bronx, and you were single digit age, you played the VG's quite a bit and your parents were a little worried it was sucking away your good time. But I am happy to read your evaluation because it shows (in your one case) that just because a kid seems hooked on video games it does not ruin him for life and even allows a balanced evaluation as you have. I am going to use your judgment in my chapter on raising a kid well in reference to the not so bad affect of video games and how much a parent should interfere or worry.

Hey Ed,

No harm there- I typically take your comments with a grain of salt (no offense). I don't play video games anymore due to lack of free time but I used to play a fair amount throughout high school and college. It's pretty standard for teens to play with their friends (sports games and shooting games are the most popular and most social).

In terms of good or bad- I can't really say either. If you're reading about how they cause violence in the real world or take away from social skills of teens. I don't see video games as too different from TV or the Internet. Too much of any of those activities is a bad thing. Parents need to do more parenting and not let their kids get sucked into that world.

I will say, though, I still enjoy a good basketball or soccer video game match with my friends when given the chance.

Grammar: Just will note I won't mention this again since you already explained. But the more I read, the more I become aware of the "shorthand" style and I must be honest--as a native English I find that this issue makes it a technically unsatisfying read and I can't help thinking this is simply a very rough draft. Okay will not repeat this again--I think you get the idea.

Characters: I found these characters--Ali and Kim much more interesting than the others in Slim Novel 1, including Kimi. This has nothing to do with their circumstances and everything to do with their inner life. Kim(ura) seems to be the most rounded-character with a sense of consistency. You present a very charming and attractive male character. Ali, while an interesting character concept, seems a bit unbelievable, especially her speech. She goes from sounding like Granny from the Beverly Hillbillies to some ultra-liberal from current times who is a visiting lecturer at Berkeley. The scene that ends at chapter 1 seems a bit odd to me. She goes through 5 paragraphs of liberal rhetoric and that inspires passion in Kim? Haha...perhaps that was your goal...I don't know. A kind of "I love when you talk like that!" moment??? It also struck me that he does not take her seriously. "Awwww....that's so cute that you're so political...." Anyway, I think you get my point and maybe this is what you wanted?

Speech/Lingo: I understand what you are trying to do with Ali's slang but I find it distracting and cumbersome to read. The idea is okay but it may be a bit much. Is she a Scarlett O'Hara or Ado Annie? A mix could be okay but the way the speech is written, it feels too disjointed and schizophrenic. Either that or she's practicing for a career as an impressionist. :). Or perhaps she's supposed to come off as kind of crazy? But it seems you want readers to think she is highly intelligent...something seems confusing here.

These two also have very odd conversations for being on a "date" or romantic situations...haha.

Inconsistencies: In chapter 2, Kim says, "Ten after! Typical American girl!" he muses. "Likes to keep a man waiting." I was under the impression that Kim finds Ali attractive because she's different from what he knows--foreign, white, blond, aggressive etc. But all of a sudden now he's an expert on typical American women? I can see him taking note of the behavior, but his comment seems out of the character's experience, unless there's info in other chapters I'm not aware of.

Descriptions: perhaps it's just my personal taste but I find that some of the physical descriptions overly detailed to the point of being clinical or mechanical. In other words, too much information, that nothing is left to the imagination. I think interesting effects can be conveyed by using inferences and some degree of mystery, rather than describing every nook and cranny and dotting every "i" and crossing every "t", but of course that could be a matter of preference.

Overall impression related to style/effect. Generally, as I read more of the Slim Novels, I'm sensing a few things. In some parts, you get into almost character/situational studies reminiscent of Somerset Maugham but in other parts, the characters and situations seem almost cartoonish...if that makes sense. Zane, for example, is so over-the-top like some character from a Carol Burnett skit. But as I said, the character of Kim seems to maintain his dignity throughout it all that makes me wonder how did such a "real" person get mixed up with all these odd characters? I actually "like" him!

26 Aug. 2014, c.afternoon, I call room number but as usual just rings.  So I call -2000 and tell his room and nurse’s station comes on and they bring him to phone.  He recognizes me at once, seems not like nursing home and will tell me more when I visit which he very much wants.  He likes me to bring deli roast beef sandwich and spontaneously asks for portable shaver and says someone stole his.  Nurse says it is about 1 hour south of Lincoln Tunnel by car and at or near New Brunswick train station.  In August 2015

01 April 2015 05;15 Tokyo but 31 March 16:15 in NJ.  Dan is now in Room 281 by the window. They get him right away but he does not recall me, Cousin Eddie. He hears it as Henry.  He does not recall Downstairs Annie, the black woman. He does not reply about his niece Joy. When I ask would you like to go back to Bronx, he says that would be nice.  Finally after I ask can I visit he says OK.  So Dan has lost recent memory but otherwise sounds like last year’s Dan.
I visit Dan 04 Aug 2015 driven from Newark Airport by Leonid.  Room 279L by window and there is a room telephone. Dan recognizes me after being awakened and seems no different from how I recall him. I leave my card and get joy’s and other nephews’s numbers.
I see Dan about 9 pm actually after hours but no problem and take photos.
I called 2 or 3 May 2016 and got floor clerk and she says Dan OK and does not have phone in room and I can come any day fr 8 am.  I plan to come 17 May on way to airport to return to Japan. Plan to bring roastbeef sando.    I saw him then last time. Could not eat sandwich.

I come for days of surgery and help.  Then continue but in 2012, 04.21 Dan more or less kicks me out after I left. Angry I copied his key.   Called 2013.04.20.6.c.8pmJT (6AM NYC) Dan 92 and 1 day. Answers after about 10 rings. Sounds good as usual. Sharp. Thanks me and says no probs. I called from EAJ now 2014, 01.05.7.22:20  (In Bronx 01.05.7.0820) and Dan picked up on 3rd ring and voice remarkably strong. I asked how things were and he alluded to some trouble and I asked what and he rambled a bit about the death of his mother, “Frances” (My Aunt Fanny) and mentioned I’d been at the house then, which was correct because he and sister called me and I examined her in bed; she had an arrhythmia and I took Dan and Renee in the kitchen and discussed. It must have been in winter 1966 because I recall I was on duty at Montefiore and in surgical scrub suit.  Dan recalled I said “She was running away from death” (I completely forgot my exact words but he is probably correct; he has a good memory for quotes) I took them into the kitchen to try to gently break to them that she was going to die not too far in future and maybe prepare them for calling ambulance.  But when we came back to her room (We may have heard a sound from it) she had fallen from the bed, dead. (Probably an agonal movement as she died).  I always felt a little guilty that I had them and myself leave her bedside just as she was about to die. Strictly speaking I should have instructed them to at once call for ambulance and not have left her side.  Anyway, Dan talked a bit more but I could not figure the problem. He was quite friendly and did not want to stop the conversation.  He asked me about the nuclear problem in Japan. I asked about Annie –“She’s OK.”  About Larry why his email stopped and Dan said “He’s traveling a lot now.”  And also the cleaning women and he said they come and he has a very good one now.  Then I good naturedly ended the call wishing him the best and saying I’d call every month.  Dan is now 92 years,, 8 1/2 months.  VG
Then he fell in late April 2014, getting out of bed to pee, I guess, at night, and hit head but managed to call 911 but they had to take the door down because no one had a key.  Then he was in Montefiore and Joy put him in the nursing home in NB NJ. Where he presumably still is. Lat tried to get him late Sept 2014. (And see above for my Aug 2015 visit to Nursing Home)   And see above Dan’s probably death in Nursing home, I see him last mid May 2016 and he is essentially vegetable and sometime after died.

Foreign Travel Tips Timing is everything when it comes to saving money on flying, or wasting it. (Illustration: iStock)
The easiest way to waste your money flying is through bad timing — both  booking your flight at the wrong time or flying at the wrong time of day or day of the week. So take the time to find out the best times to do your flying and booking.
In an in-depth analysis of plane fares, the folks at Hopper.com found that airline passengers pay about 3 percent higher fares when they book on Fridays at around 3 a.m., so they recommend you avoid late-night/early-morning ticket-shopping binges late in the week.
Hopper also says the cheapest day to leave for your trip is Wednesday (when relatively few people fly), while the most expensive day is Sunday (when just about everyone is flying to get back to work Monday). As for your return flight, Tuesday was found to be the best day for you to return from a domestic trip, while Wednesday was the best day to head home from an international trip. All told, Hopper says flying on the right days could save you an average of $85 on domestic flights and $120 for international flights.
There’s also the matter of how far ahead you book your flight. In its 2014 analysis, CheapAir.com found the best time to book domestic fares was 47 days in advance of your trip. CheapAir finds buying too late can cost you an additional $111 if you buy within 14 days of your flight, and an extra $174 if you buy within seven days. Buying too early can be expensive, too: CheapAir says tickets can be about $50 more if you jump on them when they first open for sale.
So in flying, time really is money — or money wasted if you time it poorly.

Pop, it's been a while -- thought I would update you on my activities.

 The law firm I'm with, Reed Smith, sent me to project manage a large project for one of their biggest clients, GE Capital in Norwalk CT (by coincidence my friend Cindy worked for them for years and in that location) so I was temporarily living in Stamford CT for 5 months. Just got back to Pittsburgh the beginning of April. By the way, I saw Sandy one time while I was there -- she came into Stamford to meet me for lunch one Saturday in March -- since she happened to be in NYC at that time for a couple of weeks.

Also, not sure if you heard--don't know how regularly you are in contact with family, but Aunt Bobbie passed away peacefully at home in early December -- I think December 18. They had a burial and then a gathering at Whisper Lane with friends and family officiated by BJ and Richard. I was able to attend the gathering. Jimmy was not able to come.

Due to my good work on the client project, I got a nice raise and bonus, and they are letting me go back to Chicago--Reed Smith has an office there with about 150 attorneys. So I plan to be back in Chicago for good by end of June/early July.  Also, I may possibly get a job offer from another large law firm with headquarters in Chicago. Incidentally, I think it's the same firm your female lawyer is with -- Baker & McKenzie. I've been talking to them since January about a legal project management job they are designing--they are trying to development that area at the firm--it's the latest "initiative" at law firms. They flew me into Chicago for an interviews late March and we're in negotiations. They are the largest law firm in the US with many offices internationally. But either way, I'll be back in Chicago. It will be nice to be closer to Jimmy and family and my many friends there. I do miss Chicago.

My education--I've been continuing working on my MBA in project management--online. Just finished my finals for last session. I now have 6 more courses left and I could be done by end of this year or by March 1, 2016 at the latest.

Well--hope all is well with you.  And I hope your feet/legs are better. By the way, my health seems good-lost some weight, BP stable.

Okay--I will write a "piece" for you about the school after the orientation, and how this program relates to my job now. I've done quite a bit of research in choosing this program. I think you'll find it interesting! Although it's more unusual I like the idea that I'm going to graduate school later in life.  It keeps my mind alive and I hear that's good to prevent or delay pew otential dementia! Also, I will be learning the latest stuff and I think that keeps me contemporary and progressive. At the law firm, more and more, I work with those who are younger than me and it's important to be seen as someone who is not stuck in the past--this is very common with people of my generation, so I think going to school now keeps me "fresh" and that will help my marketability as a more mature person in the workforce.

I think the key to staying competitive in the workplace as an "older" worker is to cultivate the advantages of being mature, having the general work experience and life experience, combined with a very cutting edge "niche" specialty. The beauty of a cutting edge niche specialty is that they are usually new areas so not a lot if people are involved in it yet and because it's new you don't need a lot of years of experience since it's still in development and people are making it up as they go along.  So it doesn't matter if you're young or old. You just have to have the relevant skills, which you can learn, and vision, which you can often transfer from past experience. I'm finding this is true in my current job.

Hi Pop -- a quick message.  I have found a great new 1-bedrm apartment (will send photos once I move in). It has almost everything included--heat, reserved parking spot, balcony, dishwasher, air conditioner, ceiling fans, everything in great shape and newish, near public transportation and major expressways, very nice, clean, and safe northside neighborhood--probably one of the safest neighborhoods in the city--literally.  It's actually a condo that I'm renting. All I pay is electricity.  And the rent is only $910 a month, which according to an online exchange calculator is about 93903 yen.  The low price for all that's included is apparently due to the size of the main living area, which is 12 x 11 and people think it's small and the location is a bit farther northwest for the typical person--bu great for me--I used to live just a bit south of there a few years ago and happens to be a few blocks from my dentist's office!  Bedroom is a nice size--can fit a king size bed plus other furniture. By Japan standards, probably large for one person--ha!  The whole place is 450 sq. feet.  Very cute place--great find! My address as of March 22, will be:

14 Nov 2012 Email from Jim:  I'm doing fine, a busy nephrology group practice with a bunch of personal responsibilities and blessed with a very loving wife and kids all growing too fast.  Josh just now turned 17 and very focused on school and future college, interested in biology and possibly medicine, quite the cross country runner, and piano.  Anna will be 15 December 20th and turning into a lovely young lady, very gifted in flute and insightful.  David will be 11 on March 4th and an active boy with a love of technology and gadgets, smart and great on the violin.  Kathy holds us all together and is a great school nurse at our school part time and keeps the kids in line.  I owe a lot to her in maintaining our household and family together in one piece in this crazy world
  Koko is doing fine.  She continues to work at a large law firm in Chicago, got her certificate as a paralegal and has a special position in marketing at the firm now.  She is very responsible financially now and enjoys living in Chicago and is healthy.  We just met her this past weekend which we enjoyed staying in Chicago to get our big city fix

He gets good grades in his school: advance classes in Math, History, Spanish and Honors in some other subjects. He has also got very high SAT score.

So based on his academic achievements he deserves joining a good college. He has an interest in study business-finance and therefore, he wants to go to one of the Business Schools. He now explores such schools in University of Michigan, Penn State Univ., UPenn, Univ. of Virginia, Univ. of Maryland and some others.

Except for academics he has a lot of other interests: social life with friends, sports- lacross, tennis, golf, music- just as any guy of his age. Starting this April he drives a car by himself. He is very independent in his 17.

We went to Hong Kong, Macau, Bangkok, train to Penang Malaysia, Singapore, Kathmandu Nepal, India, Amsterdam and home. We took my sisters daughter Lara for the first three weeks. She was born in Bangkok 38 years ago and has always wanted to see Thailand. We even visited the hospital where she was born. A highlight of the trip was our time in India. Delhi and Mumbai were crowded, noisy and dusty.  We and our Dutch friends rented an SUV with a driver and took a wonderful 10 day trip in the south of India from Hyderabad to Goa.

We spent 5 days in Rome in November and then took a 19 day cruise from Italy to Ft. Lauderdale with many good stops.  This is what one does when they are tired all the time and short of breadth.

Just before Rome I lost a lot of vision in my left eye which I found out (after we got home) was a Retinal Vein Occlusion. See attached photo of my retina.  I am seeing better now and have had 2 shots into my eyeball of Avastin to prevent neovascularization.

My Atrial Fib has been going on now for 1 1/2 years and I am pretty symptomatic (because of my mechanical mitral valve).  I have tried several antiarhymic meds without success. I have an appointment at the Mayo Clinic in Rochester Mn. on March 19th to attempt to fix my heart.  Deena and I leave this wednesday to drive to Mn. The specialist at the Mayo Clinic seems to be the only ones willing to try an ablation procedure on my atrial fibrillation. My last problem (sorry to complain so much) is Sleep Apnea.  The Mayo Clinic people required a sleep study prior to seeing me and I have mild sleep apnea diagnosed two weeks ago.  I began on CPAP 10 days ago and it helps a lot.  I have stopped napping.
I hope things are OK with you.  I will try to give you a follow up when we get back from the Mayo Clinic.

Please do not worry about my doing a favor; au contraire,  I value Principles of Neurology so much that I feel honored to be helping in my small way.
   My situation is: I am an 82-year-old, still-employed physician, expatriate in Tokyo, who considers himself a kind of idiot savant in that I do nothing much else than closely read medical tomes in successive editions. And neurology, one might say, is my hobby. Yes, I have done this for other texts. In immediate fact, we just finished a cover-to-cover correction-and-comment of Fundamental Neuroscience, 4th edition, at the request of its senior author, Professor Larry Squire, and emailing the corrections on the 51 chapters to his Elseviereditor in New York. Recently, I am being helped by Research Assistant Ms Yuko Sato, a university soon-graduate with an interest in editing; her 21-year-old eyes and her ability as a non native but excellent reader, speaker and writer in English, helps me much.
   Finally here, I have 2 websites Physician's Notebooks on http://physiciansnotebook.blogspot.com and The Slim Novels on http://adventuresofkimi.blogspot.com, which you may click on at your leisure, to see what my main work is.
   As I mentioned, I do not wish to bother you with emails, so if you have an editor at McGraw-Hill that you would prefer to receive our corrections, I will be happy to send them there. Oct 12 2009.1531 she is still there and trying to sue the father, Richard Gray for child support in USA. She can be interpreter. Still there 2010.05.24 and I call 1 week and maybe we all go to Habarovsk in August. And 2010 06.21.1.12N. As of late June 2010, I may see her in Khabarovsk with son and dau last week August when I go for a weekend and she’s there for 20 or so days to get dau Russian nationality.  Olga seen 12 Dec 2011 Weight w clothes no shoes after pee at 3PM 55 kg also she is Russian fasting this week. BP 112/c.70 and HR 58 on Tenormin and SaO2 98%.  Echo shows many small defects MR, AR, PR, TR all trivial and 60+% EF and atherosclerotic degen of AV and hypokinesia LV. Also good female hormone E2 90 and progesterone Our K. Also TC was 230 and she has LDL 115 and HDL in 60s.  She will have coronary angio  (Done, was normal)and holtering later this month in hosp. Stress test mildly abnormal but no obvious coronary. 16 Apr. 2013 as usual – freq messaging. Same as of 1 Dec. 2014.   Richard Kolomeetz, born 18 April 2003, blood type ?A+ At start was on Concerta 27 mg two caps a day in morn. Note on 2011.12.11. Olga writes Richard takes one cap Concerta a day. Father putative Richard W. Gray (tested and not the father but Olga thinks he bribed the test result).  Mid Sept 2010 Body Wt 31 kg, height 1.38 m and BMI 17  In 2011 Dec he is speaking more Russian and usual behavior. Reduce Concerta to one a day and continue till he is off it. Stop Concerta Feb 2012 because calmed to be  blind in 1 eye maybe from it and substitute Ziprexa and now 30 Nov. 2012 Olga reports he is 48 kg and LDL chol 224. He had great increase in appetite since Feb 2012. On 2013,November in gairai, Richard is 54 kg and 1.58 m, BMI 22 up from 17  from stopping Concerta.
Now , 30 Nov. 2014 1.65 m, ?heavier than as child.  Also Zyprexa seems to be the med that caused weight gain; he eats lots of ice cream packs also he went off Concerta so that is a good effect of being on it and the danger of going off. Today 2013.12.18 I suggested he go back on Concerta but Olga refused.
  But note he lost vision in one eye late 2011 maybe from the Concerta which I had told her previously to stop.  However this seems to have been factitious as of 1 Dec. 2014. Respiradone only occasional for tranquillizer. Advise summer camp in Khabarovsk. Olga says 2 weeks. On April 1 2012 he is taking Zyprexa and screaming and shouting all the time. In Nov 2012 I force the issue of Richard getting educated now in Russia and Olga refuses for motherly reason.

Since she's in hospital, I've been spending quite a few time with him and what I notice in his behavior is that sometimes he acts insane ( kicking, not listening to what I say at all and it can last up to an hour –suggests temporal lobe epilepsy) but most of the time he's been a good kid.  Whenever he acts crazy and whenever I get tired with dealing with him, I just say oh look there's something looking at you or whatever to just scare him and that's all. It works immediately. ( I know scaring is not a good way to calm a kid down).
Whenever I ask him to do something I try to say 'thank you' or 'good boy' or ' well done', so he feels he was really needed and appreciated. Also he hates being alone so he often calls me to sit with him and watch tv and just be there with him. He does have difficulties on concentrating and sitting still, and I think that's half genetic half due to a loud environment which his mother makes.

4. About school, he is very bad with Chinese characters. I think he would never be able to write them. He is better at reading them thand writing. Also he seems to be good at math.
He has friends and sometimes he goes out with them to a riverside and play games. He's getting along with them, I guess. He loves to play golf and I'm even secretly hoping he would become a golf player !  (Ed: his intelligence not bad, I think, I noted in 2010 he rearranged stamps in order very quickly)

5 he doesn't have a girlfriend, and he is not that interested in them. I asked him whether he had a girl he liked and he answered no.

Yuko was born 1993.05.16 from Japanese father Mr Sato – Now in 2014 age 51(his DOB 5/2/1963) - who Olga met in bar when he was computer salesman in Khabarovsk. Yuko is trilingual. She tells me first language was Russian her mother spoke to her in. Then, age 5 she went to Japanese kindergarden and quickly got Japanese and spoke it with friends. Her English started age 10 to 15 when she went to Russian school 1x a week and they taught English course. Also she had Russian student friends in Russian and from them learned about Internet YouTube and got into watching movies and stand up comedy in English. Also visited USA 3x age 7/8 ad 10/11. Her English became like now starting at Chuo 2012.  In hi school. Uses words like Bitch to her mother and Fuck you. 2011 Dec studying.
 I meet her at Suidobashi 2012.05.06. c7.14 30, go to EAJ.  She Starts Chuo U March 2012 majoring in Policy Studies (International Affairs): Courses Policy Studies, French, Statistics, English, Japanese. I advise add Russian next year.  By 29 Nov. 2014 she is 3rd year at Chuo and lives at BF’s mother’s house and comes for arbeit to me weekends and has jobs cleaning and translating.English was good from June 2012 but very good now.

Semi seen 29 July 2011 first of yr. Still alive on its back I put it in grass   In 2014, during August from 1 Aug to 26 have seen 4 and collected 2 now on my desk.
1st Semi of 2015, seen 24 July about 12:30 PM in middle of road on back.  I picked it up and found it very much alive and took it in my hands to the company flat, I had thought that the semis you find in last week July or very early August on its back in street was dying from a natural  effect as part of life cycle but this one surprised me by being very energetic and alive.  It emitted sound with low pitched vibration. It tends to cling to surfaces and even one’s skin by sticky feet and likes to be upside down on underside of tops of surfaces.  I kept it at home for several hours and it was fine and then I liberated it in the inside garden of the building.  The experience made me realize that the phenomenon of semis being found dying or dead on their backs on the streets of Tokyo each year at end of July was probably due to the existence of urban areas with pavement.  Normally in their life, they are in forest and shrubbery on the ground and they creep or crawl upside down on leafs or tree trunks or twigs.  But in the city they often land on pavement and fall on their backs and cannot right themselves and probably die of dehydration.  Also on the internet nothing mentioned about semis dying on the streets in July. Only about their irritating sound.  I never am bothered by their sound but their appearance dying on street is distinctive for Tokyo in last week July.

Muriel Tish was born July 28, 1925 and still is dividing her time between Palm Beach and NY. My brother in law passed away nearly two years ago, so though she is a widow, she is very well provided for. She has had successful hip surgery and is living on her own and doing well.
Sol Tish: 1/28/1899 to 12/15/85  My Father was born in England, January 28, 1899 and emigrated to USA when he was eleven years old, probably about 1910. He died December 15, 1985 just before his 87th birthday.

As of 2 Apr 2010, I have not spoke with him since about 2 March 2010 but I think he continues in partial remiss with WBC 20,000 abd hgb 16 gm. (2010.05.26, no contact since March 2010) 2010.12.06.1.EAJ, saw Steve 2 Dec. He had UGI hem a week or two before from possible false aneurysm splenic artery. Almost died, in ICU, released last week. WBC has been 60,000 recently and thrombos 800,000. He is clearly in relapse. Hgb 12 grams after some transfusion but apparently Fe defic. I suggest Bone Marrow transplant. 12/2010: WBC 60,000 now so I guess prognosis poor for more than 1 yr (2013.04.15 – still going strong but WBC 120,000 and Hgb OK) but he looks as usual good and can resume jet set after the strange GI bleed. His plates 800,000 and stopped aspirin after the bleed (Note the hi count did not protect against bleed. From history he first felt unpleasant fullness sto and induce vomit and bright red blood. The surgeon says a complic of the splenectomy from adhesion mass and false aneurysm splenic artery branch or stump. He drives me and the 2 Japanese nurses over 59th St Bridge to his Gatsby, we meet Claudine and Kyle and Blaise. Alexander still has 1 to 2 years before the Bar, Doug in architectural college, and Brandon pre med (Stopped it and now business in 2012). Steve will soon fly to Brazil. He nearly missed hemorrhaging on flight from HK to NYC. Addenda: hemorrhaged badly mid Dec 2010 and finally gastro-jejunostomy with blind pouch. He called me mid March 2011 from HK – traveling again and OK and WBC c 40,000. By Dec 2012 still in the partial remission with c. 30,000 WBC. I last visit 27 Oct 2012. Alex now lawyer. Others in school. Apr 2013 His WBC 120,000 last months but he feels OK and Hgb normal and he flies all over. Rai knows and treating conservatively with watchful waiting. I ask him to consult S&K on BMT. He said he would but he did not mention it after. It would have been early 2013. And now 30 Nov. 2014, Steve pissed off at me because he finked out in late Sept 2014 to drive me to Dan’s and I let him know it. But last I heard then he continues in his usual partial remission. Tofranil Day, elements of from latest experience 2012.05.20.7.2351. A 12+ year old tofranil 25 mg in bubble swallowed on 12+ hr empty stom at 1001AM at flat on futon w small amt water and did not drink usual bottle of water for the day and noted the usual short sleepiness in 1st hr and also orgasmed c +30 min and VG and no prob w erection.     Got your snail mail some weeks ago.  Yes, it is true, you kept me on your regular mail list for many years without a response from me.  Perhaps 3-5 years ago I did respond, following which mail from you stopped.
    Here is a fragmentary update.
    I am still married to Sheli and we have lived in Cambridge since 1970 and in the same house since 1980.  We have two girls who live in small towns in western Massachusetts, each with two grandchildren.   Sheli works for the Cambridge school system, teaching pre-school teachers how to teach reading skills.  She loves the mission, but as the economic crunch has squeezed the budget she has less time to do it and spends more time on soliciting funds.  She has found this increasingly dispiriting.
    I have continued to do immunology research and have taken on an increasing teaching and administrative burden.  I am only a fair researcher but my years of experience in left-wing politics have given me some skills in administration that I can use.  My major strength is a relative lack of arrogance and hubris - or at least the ability to camouflage what I have.
     My contact with Einstein and fellow students has been minimal.  I have an irregular telephone conversation with Herb Schneiderman who remains in Syracuse but is now retired.  Once in a great while I see Danny Stern. (Died Nov 2012) We all met twenty years ago to reminisce with Salome Waelsch.  A year ago Danny was in Cambridge to visit one of his daughters and we met through her.  He doesn't answer my notes.  He continues to tour giving very popular lectures on the psychiatry of child-raising.
    Shortly after her 100th birthday last year Salome died.  We went to her memorial at Einstein.  To my surprise it was quite wonderful as there were many people anxious to recall her major contributions to mouse genetics, her special courage as a German, Jewish woman scientist in the 30s and beyond.  Just as many recalled her sense of mischief and fun.
    Einstein on close inspection has stronger science, lots more money and yet is still riven with the secular/orthodox dichotomy that marked its creation.  Salome, Alex Novikoff, Lou Fraad and the Scharrers formed the secular, left pole and Yeshiva University its opposition.   The menu at the memorial featured kosher sushi.
